Arizona DPS Fingerprint Clearance Card Fingerprinting

AZ EFT Fingerprinting provides fingerprint card services for Arizona Department of Public Safety Fingerprint Clearance Card applications, including IVP and Non-IVP applications.

We provide the fingerprinting portion only. AZ EFT Fingerprinting does not issue clearance cards, approve or deny applications, determine eligibility, run background checks, or choose which card type/statute you need. AZDPS states that the applicant’s employer, licensing board, school, agency, or sponsor should tell the applicant which card type and statute/reason fingerprinted to select.

What We Provide

For both IVP and Non-IVP applicants, we can provide:

  • Completed fingerprint cards for AZDPS Fingerprint Clearance Card applications

  • Identity verification using your valid government-issued photo ID

  • Proper fingerprint capture using professional live scan equipment

  • Printed fingerprint cards for paper submission to AZDPS

  • Sealed fingerprint envelope when required by AZDPS instructions

  • Assistance making sure the fingerprint card is completed for the application type you bring in

AZDPS does not provide fingerprinting services to the public for Fingerprint Clearance Card applications. AZDPS directs applicants to use a third-party fingerprinting service or local law enforcement office for paper fingerprint cards.

What You Need Before Your Appointment

Please bring:

  • Valid, unexpired government-issued photo ID

  • Your AZDPS application/reference information, if you applied online

  • Any AZDPS paper application packet, affidavit, chain-of-custody form, or envelope provided to you

  • The card type you are applying for: IVP or Non-IVP

  • The statute/reason fingerprinted provided by your employer, school, licensing board, or agency

If you are unsure whether you need IVP or Non-IVP, contact the organization requiring the clearance card before your appointment. DPS states that it cannot tell applicants which card type or statute to select.


IVP Fingerprint Clearance Card Fingerprinting

What Is an IVP Clearance Card?

IVP means Identity Verified Prints. IVP cards are commonly required for certain education, school, childcare, and other regulated positions. The organization requiring your card should tell you whether IVP is required.

IVP Application Process

For an IVP application, AZDPS indicates that you must either apply online through a PSP account, call the Clearance Card Section to request a paper IVP application by mail, or pick up a paper IVP application from DPS. AZDPS states that IVP forms cannot be obtained through the guest paper-application download option.

General IVP steps:

  1. Create or log into your AZDPS Public Services Portal account.

  2. Select Fingerprint Clearance Card.

  3. Select Apply for a New Clearance Card.

  4. Choose IVP if your employer, school, board, or agency requires IVP.

  5. Select the correct statute/reason fingerprinted.

  6. Complete the DPS application and payment process.

  7. Follow the fingerprinting instructions provided by AZDPS in your PSP account or paper application packet.

  8. Bring your reference/application information and any DPS-provided forms or envelopes to your fingerprinting appointment.

What to Bring for IVP Fingerprinting

Bring:

  • Government-issued photo ID

  • AZDPS IVP application/reference information

  • Any IVP paper application packet, form, or envelope provided by AZDPS

  • Any instructions from your employer, school, agency, or licensing board

Important IVP Notes

AZDPS says applicants cannot get printed at the DPS office and cannot drop the IVP form back off there for this process; the applicant must take the application to be fingerprinted and mail it back as instructed.

We can complete the fingerprinting portion for your IVP application, but the applicant remains responsible for submitting the complete application package to AZDPS unless AZDPS instructions require a different submission method.


Non-IVP Fingerprint Clearance Card Fingerprinting

What Is a Non-IVP Clearance Card?

A Non-IVP Fingerprint Clearance Card is used for many Arizona employment, licensing, volunteer, and regulated-service purposes where IVP is not required. Your employer, licensing board, school, or agency should tell you whether Non-IVP is acceptable.

Non-IVP Application Process

AZDPS allows Non-IVP applicants to apply electronically through the Public Services Portal or submit the current paper application. For electronic Non-IVP applications, applicants follow the PSP instructions and may be directed to electronic fingerprinting options. For paper Non-IVP applications, applicants must use the most current DPS application and include the required fingerprint affidavit with the paper fingerprint card.

General Non-IVP paper steps:

  1. Download or obtain the current AZDPS Non-IVP paper application.

  2. Complete the application.

  3. Bring the required Non-IVP Fingerprint Affidavit to your fingerprinting appointment.

  4. We complete your fingerprints and the required fingerprinting portion.

  5. The fingerprint card and affidavit are sealed in an envelope before being returned to you.

  6. You submit the application, payment, and sealed fingerprint envelope to AZDPS as instructed.

Beginning January 2026, AZDPS states that only the most current version of the Non-IVP paper application will be accepted, and paper applications with paper fingerprint cards must include the new Non-IVP Fingerprint Affidavit.

What to Bring for Non-IVP Fingerprinting

Bring:

  • Government-issued photo ID

  • Current AZDPS Non-IVP application, if submitting by paper

  • Non-IVP Fingerprint Affidavit

  • Any AZDPS-provided instructions

  • Payment/application materials required by DPS, if you plan to mail or deliver the application after fingerprinting

Important Non-IVP Notes

AZDPS states that paper fingerprint cards will not be accepted unless they are accompanied by the correct fingerprint affidavit and properly sealed.

If you apply electronically and use the official electronic fingerprinting option, AZDPS states that electronically submitted Non-IVP fingerprints do not need the paper fingerprint affidavit.
